Welcome to the 2026 MACDC GOALs Survey, which will collect information on CDC Accomplishments in 2025!

Once you log in with the User Name and password that we sent to you via email, you will be good to go. 

Please remember to do the following:

• Before you Start, take a few minutes to read the “Instructions for Completing Your 2026 GOALs Survey,” which was attached to the email you received from us. Reading it, and referring to it when questions arise, will help you avoid mistakes in navigating the GOALs Survey.

• Please submit your completed GOALs Survey, which consists of several individual surveys, by no later than Wednesday, February 4th.

As you know by now, we make good use of the GOALs Survey data, through our reports which highlight the collective impact of CDCs, through our use of the data in our advocacy for CDCs, and as you use as one of your reporting requirements to EOHLC for your CDC recertification and CITC.
